- While we bill your insurance company as a courtesy, the patient is ultimately responsible for payment for services rendered and products purchased. The insurance company makes the final determination of coverage, and the patient is responsible for payment of all balances.
- If your plan has a deductible that has not yet been met, the approximate cost of the visit is due at the time of the exam. If your plan requires a co-pay for a specialist visit, the co-pay is due at the time of the exam. If your plan requires co-insurance payments, a statement will be sent after the insurance company determines the amount of the co-insurance due.
- All eyeglass orders are custom orders; therefore, refunds are not possible.
- Returned checks are subject to a $25 fee.
